Wellness Coordinator (WC) works in partnership with other members of the Interdisciplinary Care Team (ICT) to provide expertise in community resource availability and acquisition to support individuals with Serious Mental Illness (SMI). The WC is responsible for locating or curating community-based support to address social determinants of health (SDOH) that are negatively impacting the member experience or outcomes. The WC is a key team member within a team focused on eliminating the silos between medical and behavioral health care to create a more person-centered and individually focused healthcare experience.
Schedule: Monday – Friday between the hours of 8:00 am to 5:00 pm Central Standard Time.

Primary Responsibilities:
Use expertise and knowledge of local community resources to identify and coordinate services to meet individual member needs
Create relationships with Community-Based Organizations within assigned regional areas to facilitate access to services and identify gaps for resource development
Monitor and manage incoming referrals to prioritize and manage member needs and address high priority needs quickly to reduce risk of decline in health or hospitalization
Provide consistent communication to members and ICT on timeline and availability of requested resources
Escalate resource requests to Medical Director when member health or safety may be impacted
Assign and manage caseload sizes based on geographic/acuity guidelines
Perform other duties as assigned

Required Qualifications
High School Diploma/GED (or higher) OR 4+ years of community resource support
1 + years of experience with community resource support
Intermediate level of proficiency with Microsoft Office (Outlook), Microsoft Teams, and other web applications
Access to a designated quiet workspace in your home (separated from non-workspace areas) and is able to secure Protected Health Information (PHI)
Access to existing high-speed internet service or reside in a location that can receive a UnitedHealth Group approved high-speed internet connection
Preferred Qualifications
Bachelor’s degree (or higher) in mental/behavioral health field
Experience in managed care or health care industry
Experience obtaining resources for individuals with mental health and/or substance use disorders
Experience working in a virtual environment that requires coordinating with multiple team members
.Experience working with low-income populations
Soft Skills:
Creative problem-solving skills and ability to use community, network, and/or other tools to locate resources to meet members’ needs
Demonstrated and effective skill in organizing, scheduling, and completing day-to-day work and tasks timely to meet or exceed program metrics
Ability to handle sensitive issues with members, providers, and community-organizations in a confidential and respectful manner
Excellent customer-service, interpersonal, and problem-solving skills
Strong oral and written communication skills — specifically the ability to talk on the telephone and type at the same time
Ability to listen, collect relevant information, build rapport quickly, respond compassionately, work independently, with a willingness to learn and grow in an ever-changing environment
Accountability for delivering high-quality work product with close attention to detail
Attention to detail, as observed in day-to-day work
Excellent time management and prioritization skills
Demonstrate flexibility, ability to follow through on all tasks in a timely fashion, good attention to detail and a willingness to learn
